Nominate Awardees and Help us Recognize Excellence in Prevention.


During our Connections in Prevention Conference, we will offer four awards recognizing exceptional individuals and organizations at the forefront of integrating innovative practices in prevention, racial equity, diversity, equity, and inclusion (DEI).  This is a great opportunity to celebrate the remarkable efforts and achievements of leaders making significant impacts in public health through their commitment to innovation, collaboration, and community empowerment."

If you know someone who might be a great candidate for one or more of these awards, please nominate them by reviewing the information herein and submitting a nomination by July 1, 2024. We anticipate announcing all awardees by late July 2024.

Award Descriptions

Equity in Action Award

  • Focus: This award celebrates outstanding contributions to prevention by an individual or organization pioneering innovative practices that embed racial equity and DEI deeply within prevention strategies.

  • Criteria: The awardee must be an individual or organization that has developed novel approaches, tools, or programs that have significantly advanced the understanding or practice of prevention through the lens of racial and health equity. Considerations include any of the following:

    • Demonstrable innovation in incorporating racial and health equity principles into prevention programs.

    • Development and implementation of tools or methodologies that have visibly improved inclusivity and accessibility in prevention efforts.

    • Evidence of long-term impact on enhancing racial equity within community health initiatives.

 Collaborative Excellence in Prevention Award

  • Focus: This award recognizes an individual or organization championing collaborative efforts in prevention that bring together diverse groups to achieve significant, measurable outcomes in public health.

  • Criteria: The awardee must be an individual or organization demonstrating a sustained commitment to working together, bridging diverse perspectives, and achieving measurable impacts on communities - especially in fostering racial equity and DEI in prevention efforts. Considerations include any of the following:

    • Proof of initiating and sustaining impactful partnerships across different sectors or communities.

    • Examples of overcoming challenges through cooperative strategies, resulting in enhanced prevention measures.

    • Quantifiable results showing improvement in community health outcomes due to these collaborative efforts.

Empowerment in Prevention Award

  • Focus: This award recognizes an individual or organization that has excelled in engaging and empowering community voices in prevention work via efforts that effectively mobilize community members, especially youth, in advocating for change, implementing prevention strategies, and promoting racial equity and DEI.

  • Criteria: The awardee must be an individual or organization spearheading initiatives that have facilitated significant community-led improvements in public health or social justice related to prevention. Considerations include any of the following:
    • Innovative approaches to community engagement and empowerment in prevention.

    • Successful implementation of strategies that have led to significant community-led public health improvements or social justice advancements.

    • Demonstrated ability to inspire and involve youth in meaningful advocacy and prevention efforts.

Innovation in Action Award

  • Focus: This award recognizes an individual or organization that has shown outstanding commitment to advancing innovations in prevention efforts, particularly through integrating self-care, fostering financial sustainability, and demonstrating long-term impact in the field of prevention.

  • Criteria: Awardee must be an individual or organization demonstrating efforts to ensure that prevention work remains impactful, inclusive, and accessible to diverse populations, thereby contributing to the ongoing advancement of racial equity and DEI. Considerations include any of the following:

    • Integration of self-care practices into prevention programs, highlighting the importance of wellbeing in long-term engagement.

    • Evidence of strategies that promote financial sustainability while expanding prevention efforts.

    • Demonstrable contributions to making prevention work more inclusive, reflecting ongoing advancement in racial equity and DEI.
